SIRTE VALLE D’AOSTA: winning example of integrated territorial healthcare

The client

The Aosta Valley extends over an area of ​​3,261 km² and includes 74 municipalities, each with its own peculiarities and health needs.
The region’s healthcare company manages 3 hospitals and 4 districts, and provides assistance to approximately 125 thousand citizens.

A fundamental aspect of healthcare in Valle d’Aosta is the Territorial System, which focuses on the care and monitoring of approximately 5,000 patients.

The need

Before the collaboration with GPI, the Valle d’Aosta Region did not have a structured solution for the management of its systems, a situation that led to various inconveniences, in particular for the reporting of ministerial flows.
After conducting inspections and in-depth assessments, the client opted for the reuse of the SIRTE platform, successfully implemented by GPI for the Marche Region.

The renewal project of the Territorial Information System (SIRTE) is part of the Consip initiatives for digital health.

The challenge

The main difficulty arose from the Services’ habit of operating according to non-formalized processes, which made it complex to articulate their specific needs in a clear and detailed manner.
To overcome this obstacle, we have formed specialized working groups. And through targeted interviews, we analyzed the operational methods and information needs, focusing in particular on the essential data sets to satisfy ministerial information obligations.

The solution

The SIRTE project has significantly expanded the scope of Territorial Information Systems, extending them to Home Care and Palliative Care services, consultants, primary care and residential facilities managed by the AUSL.
In particular, it included two Centers for Dementia (CDCD) and two R2 facilities, significantly improving the management and effectiveness of health services in these areas.

In the first days of 2024, the Rehabilitation Folder for the management of the SIAR flow was activated.

The model is based on a technological infrastructure and an application architecture, safe and reliable, which guarantee the fluid and intuitive management of the processes, based on the specific needs of the patient care path, facilitate collaboration between the various actors involved and they allow in-depth data analysis for advanced governance.

The SIRTE computerized platform – Information System for the Territorial Network integrates the provision of the following services:

  • Integrated Home Care
  • Performance Care
  • Organizational Secretariat
  • Consultancy
  • Residential rehabilitation
  • Residential Management of CDCD (Dementia Centers) and R2 structures

The system includes:

  • Data generation functionality for sending ministerial flows
  • Creation of reporting and process monitoring and control systems
  • Integration with registry systems, management control, health records, hospital information systems, health records
  • Integration with the GPI platform for the management of COT – Territorial Operations Centres
  • Integration with the GPI Telemedicine platform

The benefits

Before the SIRTE platform there was no single solution for managing the process and patient records. And the systems present were based on third-party software that was not territorially oriented, or on reporting systems generated internally to the services.

The SIRTE Project has facilitated and standardized the work of operators, and has led to notable improvements in the management of healthcare processes between the 4 districts:

  • It ensures the coordination of treatment paths between the different professionals and the continuity of assistance between different organizational levels.
  • It facilitates remote patient management, because it is natively integrated with the televisit, teleconsultation and telemonitoring system.
  • It maximizes the integration between the Hospital, Territory and Territorial Operations Center platforms, making it easier for patients to access services and encouraging the exchange of health information in real time.
  • It standardizes and simplifies processes, helping to make management and administrative aspects uniform.
  • Speeds up the provision of services through the use of computerized systems, eliminating the use of paper documentation.
  • Facilitates the request for services/consultation of diagnostic data through integrations with: Order Entry of the HIS, LIS, RIS and Consultancy System.
  • Digitize the Evaluation Sheets and Reports in the local area by making digitally signed electronic documents available to the company Repository and Conservation in compliance with the regulations.
  • It documents the results and quality of healthcare interventions, through the preparation of analysis and monitoring systems that have structured, integrated and coherent data, thanks to the use of the same platform in all the territorial offices of the Healthcare Company.
  • It supports the objectives of improving the quality and appropriateness of performance.
  • Satisfies information obligations at different organizational and institutional levels, in particular. reference to SIAD, SICOF, FAR, SIAR ministerial flows thanks to the Business Intelligence services provided by OSLO (GPI Group company specialized in Data Analytics).
  • It guarantees security and respect for privacy in data management.

Evolutionary scenario / future objectives

The SIRTE platform has exceeded the Services’ expectations. The project continues in the maintenance phase until 2027. The following perimeter extensions are expected during 2024:

  • 1. Hospice – Hospice Folder with the related Ministerial Flow
    The Hospice Record integrates with Home Care and the Hospital System to promote a complete picture of the patient’s care in this delicate phase of his life. Furthermore, like the other modules of the Territorial Information System, it can provide information to the other SIRTE Project Folders.
  • 2. DSM – Mental Health Folder with the related SISM Ministerial Flow
    The DSM Folder for the management of the Territorial Mental Health Service is strongly related to the SERD Folder, with the corresponding Hospital Services and with the Residential Facilities for which it is possible to provide temporary support for the patient.
  • 3. SERD – Folder for Addiction Services with the related SIND Ministerial Flow
    The SERD Folder collects data for patient care: clinical diary, medical history, performance, evaluation of the patient and related dependencies, preparation of treatment paths and pharmacological therapy. It also allows the management of presences in Residential/Semi-residential Facilities for any support provided for in the Care Project. And it allows you to digitize the Drug Registry, which is mandatory for the service.
  • 4. Continuity of Care
    The Care Continuity Folder facilitates the mapping of the information necessary for the service and the production of Attachment M.
